Course Description

This course looks at the risk factors, signs and symptoms, medical management and then appropriate interventions for the classic triad of neurological diagnosis, Cerebrovascular Accident, Traumatic Brain Injury and Multiple Sclerosis. The course is designed to focus the home health therapist on potential complications, appropriate interventions and the diverse assortment of suitable outcome measures to use with these clients in the home health setting.

Course Objectives

  1. Identify the involved body structures and unique disease- specific pathology, including sensory motor and non-sensory motor symptoms, typical in the home care patient with cerebrovascular accident (CVA), traumatic brain injury (TBI), and multiple sclerosis (MS).
  2. Determine the likely symptom manifestation and potential stability of the condition based on record review and patient subjective history.
  3. Identify physical assessment strategies to quantify deficits and strengths in body functions through physical assessment of the patient, given the neurologic pathology.
  4. Identify appropriate tests and outcome measures to document meaningfully important changes in functional abilities
  5. Determine appropriate home therapy interventions, goals and plans, given likelihood of functional changes corresponding to the pathology of disease progression versus recovery; within the framework of the patient’s personal and environmental context.
  6. Understand the appropriate indications of intermittent skilled episodes of home care and/or utilization of Maintenance Therapy under Medicare regulations for the patient with a neurologic diagnosis.

Meet Our Presenter

Rachel Botkin, PT is a licensed physical therapist with 19 years of clinical and management experience in home health (HH), skilled nursing facility (SNF) and outpatient settings. She is currently the Owner and President of Botkin Rehab Services, LLC. She earned her Advanced Competency in Home Health in 2017. She received her Masters in Physical Therapy from MCP Hahnemann University and became board certified in Neurology in 2005. She has been an adjunct faculty member at Clark State Community College, teaching neurological rehab to PTA students since 2008. Ms. Botkin is a member of the American Physical Therapy Association and Home Health Section. She is a member of the Home Health Section Practice Committee and served on the Task Force to develop the Home Health Toolbox. She has presented at state and national conferences.
